WORK RELATED INJURIES
All injury reports must be made in writing to the District Office within twenty-four (24) hours of the occurrence of the injury. Serious or fatal injuries must be reported immediately by telephone in addition to the written report.
Industrial Accident Reporting
Any employee injured while on duty shall report immediately to the supervisor or building principal. An injury report is to be filed verbally by the supervisor or building principal during the shift on which the accident occurs and reported to the Superintendent’s Office as soon as possible. This verbal report is to be followed up with the necessary written documentation within fourteen (14) hours.
