SAFETY

Serious Injury Reports to District Office

All student injuries shall be reported to the school office and a Pupil Accident Report filled out with the original being forwarded to the District Office. A Student Insurance Claim Form (with an expiration of ninety [90] days) should also be filled out and given to the parent/guardian to take to the doctor/hospital in the case of more serious injuries.
